Chapter 68 The Patient with a Blood Disorder Discussion

Introduction

Today you are planning to perform quadrant scaling and root planning with local anesthesia on your 68 year old patient. She has moderate periodontitis accompanied with moderate generalized plaque and inflammation. The patient has recently been diagnosed with atrial fibrillation and has been prescribed coumadin. Her International Normalized Ratio (INR) is 2.8 when she presents for today’s appointment. Can you proceed with treatment? If you can proceed, how do you determine which quadrant should be your starting point? What hematologic control measures might you employ if you decide it is alright to proceed with treatment today?
